Thomas A. Sample, age 72, of Delta, OH, passed away early Tuesday morning, November 6, 2012 at the Toledo Hospital.  He was born in Youngstown, OH on August 31, 1940 to the late Laverne "L.T" Sample and Kathryn (Newsome) Sample.  On June 28, 1982, he was united in marriage to Barbara Hendrix .  Tom was a graduate of St. Ursuline High School in Youngstown.  Before his retirement, Tom worked as a retail sales manager for the Andersons in Maumee.  He was chairman of the Delta AA Group from which he also volunteered at CCNO Corrections in Stryker.  He is survived by his wife of 30 years, Barbara; children, Kathryn Welborn, William ("Joannie") Sample, Deborah (Jake) Nyhart, Phillip Sample, Lynn (August) Tischer and Keith Bauerschmidt; brother, Jack Sample; sister, Lori (Jim) Wash; grandchildren, Casey and Devyn Nyhart, Neall, Chelsea, Morgan, Erin and Claire Sample, August and Blythe Tischer; nephew, Joss; and nieces, Christina and Jenna.  In addition to his parents, "L.T" and Kathryn Sample; Tom was preceded in death by his step mother, Olga Sample and first grandchild, Mitchell Welborn.  A memorial service celebrating Tom's life will be held at 10:00 AM on Monday, November 12, 2012 at St. Paul's Episcopal Church, 310 Elizabeth St., Maumee, OH 43537.  Rev. J.
